Instructions for Visa Application

For those who need official letters to apply Chinese entry-visa for Academic Activity*, please provide the following informations and send them to Dr. X.-D. Hu via email at xdhu@amss.ac.cn before July 14, 2006.

(*) You may apply Chinese entry-visa for Sightseeing (as a tourist) at Chinese consulate/embassy in your country. In this case you don't need to fill in the form below.

 (0) Paper reference number

 (1) Official name in the passport

 (2) Nationality

 (3) Passport number

 (4) Affiliation (e.g. Universities)

 (5) Cities you are going to visit in mainland China (e.g. Xi'an, Beijing)

 (6) Tentative Arrival and departure (to and from China) dates

 (7) Place you are going to apply visa (e.g. USA, UK)

 (8) Airmail address AND Fax number where you want the letter to be sent to you.

 (*) If you have a companion, then you should also provide the above informations about him/her.
